ABSTRACT

Objective@#To observe the clinical effect of Jiawei-Xiaoxianxiong decoction combined with conventional western medicine for the refractory angina pectoris with Qi stagnation and blood stasis syndrome and to explore its mechanism.@*Methods@#A total of 98 patients who met the inclusion criteria were divided into two groups according to the random number table method, 49 in each group. The control group was treated with conventional therapy. The study group was treated with Jiawei-Xiaoxianxiong decoction on the basis of the control group. Both groups were treated for 4 weeks. The symptom of angina pectoris was scored before and after treatment. The Serum level of cardiotrophin-1 (CT-1), cytochrome C (Cyt C), soluble intercellular adhesion molecule-1 (sICAM-1), von Willebrand Factor (vWF), MDA and SOD were measured by ELISA, and the clinical effect was evaluated.@*Results@#The total effective rate was 75.5% (37/49) in the study group and 63.3% (31/49) in the control group. There was significant difference between two groups (χ2=5.235, P=0.022). After treatment, the level of CT-1 (105.19 ± 12.04 ng/L vs. 158.25 ± 14.18 ng/L, t=19.967), Cyt C (0.53 ± 0.11 ng/L vs. 0.81 ± 0.15 ng/L, t=10.537) in the study group were significantly lower than those in the control group (P<0.01); the serum level of sICAM-1 (261.13 ± 38.40 μg/L vs. 415.55 ± 42.26 μg/L, t=18.930), vWF (105.22 ± 18.63 μg/L vs. 145.57 ± 21.18 μg/L, t=10.013), MDA (4.80 ± 0.59 mol/L vs. 6.21 ± 0.87 mol/L, t=9.389) in the study group were significantly lower than those of the control group (P<0.01); the SOD (12.07 ± 1.84 mol/L vs. 10.89 ± 1.65 mol/L, t=3.342) level in the study group was significantly higher than that of the control group (P<0.01). The number of attacks, duration, degree of pain, nature of pain and nitroglycerin dosage in the study group were significantly lower than those in the control group (t=9.711, 11.521, 8.818, 13.922, 17.791, respectively, all Ps<0.01).@*Conclusions@#The Jiawei-Xiaoxianxiong decoction combined with routine western medicine therapy can improve the symptoms of angina and clinical efficacy in patients with refractory angina pectoris of Qi stagnation and blood stasis syndrome. It is speculated that its mechanism may be related to anti-myocardial cell apoptosis, alleviating vascular endothelial injury and antioxidation.

ABSTRACT

Objective To investigate the influence of different dose of atorvastatin on ventricular remodeling and platelet activation in patients with acute myocardial infarction(AMI).Methods From December 2014 to October 2016,170 patients with acute ST segment elevation myocardial infarction in the Central Hospital of Shan County were chosen and randomly divided into control group(low dose atorvastatin)and research group(high dose atirvastatin). The platelet index before operation and 48h after operation,echocardiographic index 7d after operation and 6 months after operation,adverse reaction and major adverse cardiac events (MACE)were compared between the two groups. Results Before operation,the platelet index between the two groups had no statistically significant differences(t=0.549,0.376,0.250,0.098,all P>0.05).At 48 h after operation,the mean platelet volume (MPV),platelet-large ratio (P-LCR),peripheral blood platelet (CD62p),activation of Gp IIb/IIIa complex (PAC-1)of the research group were (12.93 ±1.74)fL,(29.05 ±5.79)%,(45.29 ±5.23)%,(74.53 ±5.52)%,which were lower than those of the control group[(14.21±1.82)fL,(30.94 ±6.15)%,(48.37 ±4.82)%,(78.49 ±5.76)%],the differences were statistically significant (t=4.687,2.063,3.993,4.576,all P<0.05).In the research group,62 cases (72.94%)were TMPG good,49 cases(57.64%)in the control group,the difference between two groups was statisti- cally significant (χ2=4.387,P>0.05).Echocardiographic indicators 7d after operation between the two groups had no statistically significant differences(t=0.160,0.467,0.448,0.745,all P>0.05).Six months after operation,the left ventricular ejection fraction (LVEF)of the research group[(50.11±3.79)%]was higher than that of the control group[(48.45 ±4.10)%],the left ventricular end diastolic volume (LVEDV)[(110.13 ±8.83)mL],left ventricular end systolic diameter (LVESV)[(60.18 ±6.07)mL],left ventricular mass index (LVMI)[(101.52 ± 4.33)g/m2]of the research group were lower than those of the control group [(112.84 ±8.46 )mL,(63.51± 5.51)mL,(103.94 ±4.19)g/m2],the differences were statistically significant (t=2.043,3.745,3.703,2.741,all P<0.05).The incidence rate of liver damage and myopathy between the two groups had no statistically significant difference(χ2=0.525,0.000,all P>0.05).There were 6 cases of MACE occurred in the research group,16 cases occurred in the control group,Kaplan-Meier survival analysis showed cumulative non MACE event survival rate of the research group(92.94%)was higher than that of the control group(81.17%),the difference was statistically significant (Logrank=0.411,P<0.05 ).Conclusion High atorvastatin dose can better improve the platelet and ventricular remodeling,and with good safety and prognosis.

ABSTRACT

Objective To evaluate the diagnostic value of the common test parameters in acute fever without obvious infection focus and sick appearance in children under 5 years.Methods The hospitalized children with fever duration less than 7 days, anal temperature higher than or equal to 38°C, age younger than or equal to 5 years, and without obvious infection focus and sick appearance were recruited, we investigated the diagnosis value of common test parameters including C-reactive protein (CRP), procalcitonin (PCT), the white blood cell count (WBC), and neutrophil percentage (N%) , according to the ifnal diagnostic.Results Of 228 children, 42 children (18.42%) had serious diseases, the difference of CRP, PCT between serious diseases group and non-serious diseases group were statistically signiifcant (P<0.001). The diagnostic cut-off point of CRP was 67.1 mg/L by speciifcity of 0.810 and sensitivity of 0.715, that of PCT was 0.505 ng/L by speciifcity 0.762 and sensitivity 0.672. The speciifcity and sensitivity combining CRP with PCT was respectively 0.918 and 0.617. Of 228 children, 32 children had viral infections, 40 children had bacterial infections, 15 children had mycoplasma infections. The difference of CRP, PCT, WBC, and N% among three groups were statistically signiifcant (P<0.01).The cut-off point of CRP was 38 mg/L by sensitivity 0.900 and spec-iifcity 0.813, that of PCT was 0.450 ng/L by sensitivity 0.700 and speciifcity 0.812, and the speciifcity and sensitivity combining CRP with PCT was respectively 0.965 and 0.630, to distinguish bacterial infections from viral infections. The diagnostic cut-off point of CRP was 80.75 mg/L by sensitivity 0.700 and speciifcity 0.933 distinguishing bacterial infections from mycoplasma infections.Conclusions The parameters CRP and PCT have the diagnostic value for the children with the acute fever and age younger than or equal to 5 years and without obvious infection focus and sick appearance in etiology and serious diseases, espe-cially the value of combining CRP with PCT is better.

ABSTRACT

Objective To discuss the distribution of coagulation markers in patients with advanced non-small cell lung cancer (NSCLC) and the correlation of these markers with the prognosis of the patients, the TCM syndrome and the long-term thrombotic events. Method Hospitalized NSCLC patients were selected for retrospective investigation and the coagulation markers data were analyzed with SPSS13.0. Result There were 10 cases (38.5%) and eight cases (30.8%) with one or more abnormal coagulation markers in the non-advanced NSCLC group, while there were 95 cases (71.4%) and 77 cases (57.9%) in the advanced NSCLC group. ABSTRACT

Objective To observe clinical effects and side reactions of Huayu pill combined with TC (Paclitaxel+Carboplatin) chemotherapeutic regimes on treating epithelial ovarian cancer. Methods All 60 patients with the epithelial ovarian cancer were randomly divided into the treated group (Huayu pill combined with TC chemotherapeutic, n=30) and the control group (TC chemotherapeutic regimes, n=30). ABSTRACT

Objective To investigate the protective effects of morphine preconditioning on the lungs against ischemia-reperfusion (I/R) injury and the possible mechanisms.Methods Twenty-four Japanese long-ear white rabbits weighting 2.5-3.0 kg were used in this study. The animals were anesthetized with intramuscular atropine 0.5 mg and intravenous 3 % pentobarbital 30 mg ?kg-1, tracheostomized and mechanically ventilated ( VT 10 ml?kg-1, RR 30 bpm, FiO2 100% , PEEP 1 cm H2O) . Right carotid artery was cannulated for BP monitoring and blood sampling. An elastic band was placed around the hilum of left lung via thoracotomy to perform lung ischemia. Body temperature was maintained at 36-38℃( rectal) . The animals were randomly allocated to one of 3 groups (n = 8 each) : (1) Sham group; (2) I/R group was subjected to 2 h in situ left hilar occlusion followed by 2 h reperfusion and (3) morphine preconditioning group received morphine 4 mg?kg-1 via pulmonary artery 30 min before I/R. Mean pulmonary artery pressure (MPAP) and peak inspiratory pressure (PIP) were monitored and recorded. Arterial blood samples were taken before occlusion of lung hilum (baseline) and at 5, 30, 60, 90 and 120 min of reperfusion for blood gas analysis, and determination of plasma endothelin-1 concentration. The animals were killed at the end of 120 min reperfusion. The lungs were removed for determination of lung water content (W/ D ratio), percentage of neutrophils in the broncho-alveolar lavage fluid ( BALF) and microscopic examination.Results MPAP and PIP were significantly lower while PaO2 was significantly higher at 30, 60, 90 and 120 min of reperfusion in morphine preconditioning group than in I/R group. ABSTRACT

Objective To examine the influence of serum on pulmonary surfactant (PS) function.Methods Caesarean section was performed on 26-day pregnant Japanese white rabbits (full term=31 days). The pre-term rabbits weighing 20-34g were anesthetized with intraperitoneal(ip) pentobarbital sodium 0.5mg. Tracheotomy was performed. Test liquids were prepared by suspending PS in rabbit serum or acetate Ringer solution at concentration of 2.0,4.0,6.0,8.0,10,12,24 mg/ml. The surface tension of test liquids was measured by pulsating bubble technique. Test liquid 10ml/kg was instilled in the lungs of PS-deficient pre-term rabbits via tracheotomy cannula.Animals were ramdomly divided into 4 groups according to the test liquid used: group A: 24mg/ml PS suspended in acetate Ringer solution; group B: 24mg/ml PS suspended in serum; group C: 4.0mg/ml PS suspended in acetate Ringer solution; group D: nothing was instilled. Rabbits were placed in plethysmograph. Siemens-Elema ventilator was used for mechanical ventilation. PIP was set at 22.5cmH 2O, PEEP=0,respiratory rate=40 frequency/min, I:E=1:1,FiO 2=100%. Tidal volume was measured at 5,10,15,20 min after the start of mechanical ventilation.Results When concentration of PS suspended in acetate Ringer solution was 2.0 mg/ml, the minimum surface tension (?min) was less than 3.0 mN/m; when suspended in serum, ?min was significantly higher than 30 mN/m (P15 ml/kg, but in group B the mean tidal volume was

ABSTRACT

Objective The reconstituted pulmonary surfactant (SP) is composed of synthetic lipid(SL) and surfactant- associated protein C (SP-C) . The purpose of this study was to determine the surface tension reducing activity of the reconstituted surfactant containing different concentrations of SP-C.Methods Pulmonary surfactant (SP) was isolated from alveolar lavage fluid of fresh porcine lungs by centrifugation, extraction and precipitateon. SP-C was extracted from PS and SL was prepared by mixing dipalmitoyl-phosphatidyl choline (DPPC), dioleoyl-phosphatidyl choline (DOPC) and palmitoyl-oleoyl-phosphatidyl glycerol (POPG) in the weight ratio of 60: 20: 20. Three synthetically reconstituted surfactants RS-1, RS-2, RS-3, were prepared by adding different concentrations of SP-C to SL. The end concentration of SP-C was 1% , 2% and 3% respectively. Five test liquids (PS, SL. RS-1, RS-2, RS-3) were prepared by suspending the materials mentioned above in nonnal saline. The dynamic surface tension of the five test liquids were measured by a pulsating bubble technique. Thirty-six PS-deficient premature rabbits were studied. The premature rabbits were delivered by cesarean section on the 26 th day of pregnancy. The animals were tracheostomized and mechanically ventilated. 0.1 ml of test liquid PS (group Ⅱ) , SL(group Ⅲ) or RS-3(group Ⅳ) was instilled into the lungs via the tracheostomy cannula. In control group (group Ⅰ) no test liquid was instill. The animals were placed in a body plethysmograph and mechanically ventilated (the maximum inspiratory pressure = 25 cm H2 O, RR = 40 bpm, I: E = 1:1, FiO2 = 100 % made of ventilation IPPV) . Tidal volume was measured at 5, 10, 15 and 20 min after onset of IPPV. Results The minimum surface tension (?min) of PS was 0.9?0.3 mN?m-1 , and of SL was 22.6 ?1.3 mN?m-1 . The ? min of RS-3 was 0.7?0.1 mN ?m-1. The mean tidal volume after 20 min of mechanical ventilation in group Ⅱ(PS) and groupⅢ(RS-3 ) was 25 ?7 ml?kg-1 and 25 ? 4 ml?kg-1 respectively, significantly larger than that in control group (Ⅰ ) and group Ⅲ (SL) . Conclusion The surface tension-reducing activity of SL alone is low. Adding SP-C to SL greatly enhances the surface tension-reducing activity of reconstituted surfactant (RS).

ABSTRACT

Objective: To develop a method to determine Astragaloside IV in spray-dry of Radix Astragali by HPLC-ELSD. Methods: ELSD was used to determine directly Astragaloside IV in spray-dry of Radix Astragali on Elite-OSD column, using CH 3CN-H 2O(36∶64) as a mobile phase. The flow rate was 1.0 ml/min. The temperature of drift tube for ELSD was 105 ?C and the flow rate of N 2 was 2.84 ml/min. Results: The recovery of the added sample was 91.68% and RSD 1.64%. Soluble amylum was helpful for spray-dry but it can influence was determination of Astragaloside IV. Conclusion: The method is accurate and can be used in the determination of Astragaloside IV in spray-dry of Radix Astragali. Better adjuvant should be found.
